How ADHD causes issues with focus & attention

Perhaps the most obvious and well-known symptom of ADHD, difficulty managing focus and attention is such a hallmark feature of ADHD that it's almost a cliche. So, why does it happen?

People with ADHD tend to have lower-than-average levels of dopamine, an important brain chemical that regulates attention and motivation and acts as a "reward" in the brain when we do something fulfilling. Since ADHDers typically have less dopamine to work with, there is less of an inherent mental reward for staying on task, so it can often make focus, attention, and motivation more difficult.

An important distinction to make here is that people with ADHD are not incapable of paying attention, but rather that ADHD makes it difficult to regulate their attention. In practice, this means that without treatment or strategies, a person with ADHD will have trouble directing their attention on demand. So, whichever stimulus happens to be the most personally stimulating will usually capture an ADHDer's attention.

This difficulty regulating ADHD focus is a double-edged sword. More than just starry-eyed space cadets, people with ADHD sometimes slip into a state of hyperfocus. Hyperfocus causes ADHDers to be intensely focused on a given stimulus or task, perhaps finding it difficult to break away from the object of their attention. When hyperfocus lines up with an important goal, it can even be a positive side-effect of ADHD, as it allows a person to work diligently with singular attention.

However, hyperfocus can also cause problems of its own, such as causing people to neglect important self-care tasks like eating and sleeping. It could also lead to forgetting other important obligations to occupy a person's attention on something that distracts from another important goal like work or school.
